HSHS St. Joseph's Hospital Breese issued the following announcement on Sept. 23.
HSHS St. Joseph’s Hospital Breese has launched the Almost Home program. Almost Home is a transitional care program designed to help patients heal faster, rest easier and make the transition to return home as soon as possible.
Patients who qualify for the program must be physically impaired or have a medical complication affecting previous level of function, medically stable for the last 24 hours, be able to follow directions, be able to tolerate intense level of therapy and treatment in a rehab program and be able to demonstrate improvement in a reasonable amount of time.
The Almost Home program includes:
Physical, occupation and speech therapy
All-private patient rooms
Hospitalist available 24/7
Around-the-clock on-site nursing
Family inclusion in patient care plan development
“St. Joseph Almost Home Program is another way to ensure our patients are taken care of and made to feel as comfortable as possible,” says Zach Yoder, CNO at HSHS St. Joseph’s Hospital Breese. “Returning home after a long stay in the hospital can be challenging for many. This program is a great way to transition home successfully. Having the care of a nurse still available and including the family in the patient care plan development, is a great way to make sure our patients and their families receive the best type of recovery everyone deserves.”
